My first shift began at a university hospital where doctors are busy moving around with many emergency patients and general patients. In particular, there are many emergency patients and many critically ill patients in the thoracic surgery department. Every surgery is a major operation, and I am the only resident in a department that many medical students avoid. That is why I had to move around even busier, even though it was my first shift.

The patient's lips, as he spoke of his symptoms, were turning blue, and I suspected cyanosis for a moment and asked the patient to give me his hand. The patient stretched out his hand without any suspicion, and even his fingernails had turned blue.

After seeing patients, I headed to the intensive care unit when I had some time left. Since more than half of the thoracic surgery patients were critically ill and in critical condition, the intensive care unit was full. While I was checking on the patients, a nurse called me urgently and I went over there.

 

 

"This patient has pneumothorax. Please insert a chest tube."

 

"Huh? Chest tube insertion?"

 

"Yes, it's urgent because it's an emergency."

Chest tube insertion is a difficult procedure that can usually be done without the supervision of a professor starting from the second year of residency. However, this is my first time working as a first-year resident, and it is my first time inserting a chest tube on my first emergency patient. Of course, I have seen it on video, but it is not much different from actually doing it. If this procedure is done wrong, it can be a big problem, but since it is urgent, I had no choice but to start filming.

 

It is not a very big or dangerous procedure performed under local anesthesia, but it is a procedure that must be performed under the supervision of a professor. However, Professor Kim Seok-jin was not by my side, and it was an emergency patient, so it was an inevitable choice.

 

* Local anesthesia: An anesthesia method that blocks sensation in a specific part of the body while the patient is conscious, so that they do not feel pain.

 

 

So I had a chest tube inserted, and fortunately, it seemed to have gone well. However, regardless of the fact that it went well, the procedure was one that a first-year resident should not have done alone. I naturally knew that fact, and eventually, I ended up hearing it from the professor. That's how the professor's low, sharp voice got stuck in my ear.
